A NEW Ross-on-Wye project will build support networks for older people with declining health thanks to a £153,168 award from the Big Lottery Fund.
Age Concern Ross-on-Wye and District is one of 11 projects to share in £2,370,157 in grants awarded from BIG's Reaching Communities programme across the West Midlands.
The charity will work with Ross Community Hospital to identify those patients who need help after discharge and will also take referrals from family or friends concerned about the health and wellbeing of older people who find it hard to look after themselves properly.
Support workers will assess their needs and organise a network of help such as arranging medical appointments, repairs around the home, footcare, visits and outings and applying for allowances.
Janet MacDonald, Chief Officer, Age Concern Ross-on-Wye and District, said: "Older people are often feeling anxious after spending some time in hospital. They may have lost confidence or are afraid of being unable to cope, or perhaps are weak after surgery. Our new project will build a support network as quickly as possible and help them to access vital services so they can live independently at home. We will also encourage those who become more independent to join in community activities and support them to attend.
