A 42-YEAR-old man with a history of drug abuse died at his home in Leominster, an inquest heard.
Justin Harris was found by a friend on his bedroom floor with drug paraphernalia around him on November 30 last year at Waterworks Lane.
He was confirmed dead at the scene.
A toxicology report revealed previous use of cannabis and heroin and there was cocaine and methadone in his system.
A post mortem revealed Mr Harris died from bronchopneumonia due to infective endocarditis, which was caused by intravenous drug use.
The inquest at Herefordshire Coroner's Court heard his drug abuse dated back to at least 2002.
Kate Blazey from Addaction said in August last year Mr Harris failed to attend an appointment for prescription methadone for three consecutive days.
His prescription was then stopped as the level of methadone would need to be re-assessed and given at a lower dosage as he had been without it for three days.
Mr Harris sent text messages to Addaction and said he wanted to change key-worker and make an official complaint as he would like to see the methadone prescription service return to the NHS.
He did not attend any further appointments and he was discharged from Addaction on September 20.
Dr Blazey said: "I am aware there has been an increase of cocaine and crack cocaine use in the area of Leominster. This may have had some impact on his treatment."
Coroner Mark Bricknell recorded a drug related death.
