The crash happened at 10.30am on Saturday at the junction with the B4218 and A449 at Chances Pitch between Ledbury and Malvern, Hereford and Worcester Fire and Rescue Service said.
The crew from Ledbury said firefighters removed the car door to help West Midlands Ambulance Service and Midlands Air Ambulance personnel to give treatment.
An environmental grab pack was also used to reduce the impact of leaking vehicle fluids.

Firefighters said the motorbike rider, a 28-year-old man, was treated for injuries not believed to be life-threatening and two further casualties from the car were assessed on the scene and believed to be uninjured.
The road was closed for around three hours and firefighters apologised for delays drivers might have experienced and thanked them for their patience.
