A PATIENT brandished a knife at mental health staff in Hereford, according to a Care Quality Commission (CQC) report.
The patient, who was at the Stonebow Unit in Hereford, knocked on the door of a portacabin in the night and threatened staff with a knife.
The CQC report, published on January 19, said: "In Herefordshire, one reported incident concerned staff safety.
"The crisis and home treatment were already a team and they remained within their base in the Stonebow Unit until August 2022. They were temporarily moved to the portacabins while building work was undertaken.
"A patient had knocked at the portacabin in the evening and threatened staff with a knife.
"They had requested a perimeter fence and were still waiting for this.
"Staff now worked in the hospital reception area until a security fence was in place."

A spokesperson for the Herefordshire and Worcestershire Health and Care NHS Trust, which runs the Stonebow Unit, said: "We take the safety of our staff and patients very seriously.
"We have processes in place to review the circumstances surrounding any incident, and take action to prevent similar incidents occurring."
The CQC report said that incidents like this were appropriately documented and dealt with by the trust.
