THE mother of an eight year old girl in Hereford does not know whether to be amused or confused about questions in a letter from her GP surgery.
The letter, addressed to the little girl, said a flu jab clinic for all under-16's was being held at the leisure centre later in the month and she was invited to attend.
Two questions on the form wanted to know her height and weight.
Two more asked whether she was a current smoker, or had she ever smoked.
"If you are unsure of the answers there will be a health care assistant available on the day to help you," said the letter.
The non-smoking mother said her eight-year-old was far too young to be interested in cigarettes - but asking her such questions had made the girl wonder whether she should be.
